Dog Application

Thank you for taking an interest in one of our available dogs! Please fill out the adoption survey below, and a member of the GHHS team will be in touch!

Completing an online application DOES NOT RESERVE or COMPLETE ADOPTION for a pet from our shelter. It only allows you to save time when you visit our shelter. We do not “reserve” pets or hold pets. All adoptions are completed in person at the shelter on a first come first serve basis.

The shelter can be a stressful place. It takes roughly 3 months for dogs to decompress in a new home environment. 
Have you ever surrendered an animal to a humane society or animal shelter before?
Have you researched the breed / breed mix to ensure that the dog is a good fit for your household? Understanding the breed that you are interested in prevents rehoming:
List all of the current animals in your household. Please include the following: 
 
  • Species (Dog, Cat, Other)
  • Age
  • Breed
  • Are they Fixed
  • How long You've Had Them
